Sex-Based Misconduct/Title IX Complaint Form

This form is used to report information necessary to initiate an investigation of alleged sex discrimination or retaliation, pursuant to IECC's Prohibiting Sex Discrimination Policy (100.42). If you do not wish to file this online form you may contact the IECC Title IX Coordinator:

I understand if I knowingly provide false information in connection with this complaint, I may be subject to disciplinary action under the IECC Student Code of Conduct. This submission shall serve as a release of information and consent to review  all records that are related to this complaint. I further understand that if I file this report anonymously, IECC’s ability to conduct an investigation into the particular incident or pursue disciplinary action against the alleged perpetrator(s) may be diminished.
