Student Information Update

Students should use the Student Information Update Form to make corrections or updates to their personal profile information.

Changes that require documentation evidencing proof are identified below. Only one of the documents is required and may be a copy, scan, or fax. 

Proof of Name Change  
Birth Certificate
Marriage License
Passport
Citizenship Papers
Driver's License
Court Document
Social Security Card
Alternative documentation may be accepted at the discretion of the Registrar (or designee). 

Proof of Address Change 
Valid Driver's License 
Valid Vehicle Registration
Voter's Registration Card
Property or Income Tax documents
Document (i.e. high school transcript) 
Statement by the student certifying his/her address. (Correspondence will be sent to the address for verification purposes.)
Alternative documentation may be accepted at the discretion of the Registrar (or designee).

Social Security Number Change/Correction  
Social Security Card

Legal Sex Change
Valid Driver's License or Passport reflecting the legal sex
Court order or other document legalizing the change
