Community Health Coaching Program

A collaborative initiative between Crawford Memorial Hospital and Lincoln Trail College preparing students for health professions while serving our community.

 

What is the Community Health Coaching Program?

The CHCP is a collaborative program between Crawford Memorial Hospital's Community Care Network and Lincoln Trail College for students interested in Health Professions. Student participants will benefit from a rigorous training program which enables them to work, under close medical supervision, as Health Coaches with carefully chosen patients in Crawford County who need companionship and encouragement in developing healthier lifestyles.

Research shows that this type of program keeps unnecessary ER visits to a minimum, provides companionship and allows for weekly checks on patients leading to improved health benefits and early detection of potential health issues.

After completing the program, students will have the opportunity to continue health coaching by applying to CMH's paid internship program, gaining valuable experience in the healthcare field.

Fall 2025 HEA 2299: 1 Credit, 15 Hours
Classes held Thursdays, October 16 - December 18, 2025
1:00-3:00 pm at Lincoln Trail College

Interviews

30-minute virtual interviews with candidates.

 
Early October 2025

Admission Notifications

Students admitted into HEA2299 will be notified.

 
October 16 - December 18, 2025

Fall Training

Weekly lectures/classes (HEA 2299) at LTC taught by healthcare professionals including physicians, nurses, dietitians, and others. Classes held Thursdays 1-3pm.

 
December 15, 2025

Finals Week

Completion of HEA2299 coursework and final assessments.

 
January 2026

Health Coaching Skills Orientation

Full day Health Coaching Skills Competency Orientation at CMH/Crawford Room and pairing with patients under direct supervision.

 
Summer 2026

Health Coaching & Paid Internship Opportunity

Students begin working with assigned patients, visiting once per week and reporting back to the professional team. Students who successfully complete the program can apply to CMH's paid internship program to continue health coaching.

Program Requirements

Who Can Apply?

  • Be graduated from high school
  • Have a 3.0 GPA

Application Process:

  • Apply to Lincoln Trail College via online application
  • Complete CHCP Application by September 19, 2025 via this form
  • Submit one-page letter of intent clearly stating reasons for pursuing this program, including future goals and why you are a strong candidate
  • Submit letter of recommendation from a teacher, school counselor, coach or other non-relative
  • Submit transcripts (3.0 GPA required)
  • Complete a 30-minute virtual interview (scheduled during the week of September 29th)

Program Schedule:

  • 10 students will be selected for the Fall HEA2299 cohort
  • Students admitted into HEA2299 (1 credit) will be notified by early October
  • Admitted students will attend Thursday classes October 16 - December 18
  • At the conclusion of HEA2299, students will then be qualified to be a CHC
  • In January 2026, CHC students will complete a full day Health Coaching Skills Competency Orientation
  • Health Coaching will begin in Summer 2026

Following the Fall 2025 CHC session, students will have the opportunity to continue health coaching by applying to CMH's paid internship program. This provides valuable work experience and potential career opportunities in healthcare.
