The Community Health and Wellness Coach Program (CHCP) is a collaborative effort between Crawford Memorial Hospital and Lincoln Trail College designed to train students to become Health and Wellness Coaches. This program provides comprehensive training and hands-on experience in helping community members achieve their health goals.
Students selected for the program will receive professional instruction from healthcare experts, work directly with patients in the community, and have the opportunity to continue as paid interns. This program offers invaluable experience for those interested in pursuing careers in healthcare, nursing, or related fields.

Spring 2026 HEA 2299: 1 Credit, 15 Hours
Classes held Tuesdays and Thursdays, March 17 - May 5, 2026
8:00-9:00 AM at Lincoln Trail College

Program Timeline

30-minute virtual interviews with candidates.
Admission Notifications
Students admitted into HEA2299 will be notified.
Spring Training
Weekly lectures/classes (HEA 2299) at LTC taught by healthcare professionals including physicians, nurses, dietitians, and others. Classes held Tuesdays and Thursdays 8-9 AM.
Health and Wellness Coach Skills Orientation
Health and Wellness Coach Skills Competency Orientation at CMH/Crawford Room and pairing with patients under direct supervision.
Health and Wellness Coaching & Paid Internship
Students begin working with assigned patients, visiting once per week and reporting back to the professional team. Students who successfully complete the program can apply to CMH's paid internship program to continue health and wellness coaching.
Program Requirements
Who Can Apply?
- Must be 18 years old by March 17, 2026
- Have a 3.0 GPA
Application Process:
- Apply to Lincoln Trail College via online application
- Complete CHCP Application by February 20, 2026 via this form
- Submit one-page letter of interest clearly stating reasons for pursuing this program, including future goals and why you are a strong candidate
- Submit letter of recommendation from a teacher, school counselor, coach, or other non-relative
- Submit transcripts (3.0 GPA required)
- Complete a 30-minute virtual interview (scheduled during February 23-27, 2026)
Program Schedule:
- 10 students will be selected for the Spring HEA2299 cohort
- Students admitted into HEA2299 (1 credit) will be notified by early March
- Admitted students will attend Tuesday and Thursday classes March 17 - May 5, 2026
- At the conclusion of HEA2299, students will be qualified to become a Health and Wellness Coach
- Health and Wellness Coaches will complete a Health and Wellness Coach Skills Competency Orientation in May 2026
- Health and Wellness Coaching will begin immediately after orientation

The CHCP program provides comprehensive training on health and wellness coaching, patient care, and healthcare principles. You'll learn from various healthcare professionals including physicians, nurses, and dietitians. The program combines classroom learning with hands-on experience working with patients in the community.
The program is limited to 10 students per cohort to ensure quality training and individualized attention.
You'll be paired with carefully chosen patients in Crawford County who need companionship and encouragement in developing healthier lifestyles. These patients are selected by healthcare professionals at Crawford Memorial Hospital.
There is no cost to admitted students to take the 1 credit CHCP HEA 2299 LTC spring course. This makes the program accessible to all qualified candidates.
After completing the program, students can apply to CMH's paid internship program to continue health and wellness coaching. This provides valuable work experience and potential career opportunities in healthcare.
